Welcome
to Pi Beta Phi Elementary School. We are dedicated to the
academic success of each and every student. We hop you enjoy your
time at Pi Beta Phi. Below you will find all the information you
need to register your child at any public school in the state of
Tennessee.
When you come to Pi Beta Phi Elementary
School to register, please
bring the following:
1. Child's birth certificate (a certified
copy for school records, not
the hospital copy).
2. Child's social security number.
3. Health Insurance Provider
4. (Grades 1-12 only) Provide the name, address, and phone number of
the school(s) where your child previously attended or provide report
cards or withdrawal forms.
5. The permanent Tennessee Child Health Record available from your
doctor or Health Department consisting of two parts:
- IMMUNIZATIONS: A permanent
Tennessee Child
Health Record form showing the complete dates (month, day, and year)
immunizations were administered.
- HEALTH EXAMINATION: A physical
examination is required for all children entering Tennessee Schools
(K-12).
6. IMMUNIZATION/PHYSICAL EXAM INFORMATION:
The Tennessee State Law
requires that each child entering a Tennessee school for the first time
have a physical examination and be immunized against diphtheria,
whooping cough (pertussis), tetanus (DPT), polio, rubella (German
measles), rubeola (red measles, mumps (MMR), and Hepatitis.
7. DPT: 4 or more doses, one of which was given on or after the fourth
birthday. If age seven or older three doses are required.
8. POLIO: Four (4) doses or OPV or IPV. If the 3rd dose was given on or
after the fourth birthday, the 4th dose is not required. However, if a
combination of PV/OPV (2IPV/2OPV) is used, all four doses are needed
regardless of age.
9. MMR: Two (2) doses, given on or after the first birthday required,
for entrance into grades K, 4, 8, and 12. The second dose should be
administered at least 30 days following the first dose.
10. HEPATITIS B: Three (3) doses, for entrance into kindergarten
(effective July1, 1999). Hepatitis B is required for entrance into
Kindergarten (effective at the beginning of the 2002 school year).
11. VARICELLA (Chicken Pox): one (1) dose or a parental/physician
history of chicken pox is required for entrance into Kindergarten
(effective at the beginning of the 2002 school year).
12. Immunizations may be obtained from your physician or the Sevier
County
Health Department. A parent or legal guardian must accompany a child
needing immunizations. Take all records of past immunizations to the
Health Department or to your physician when completing immunizations.
The permanent Tennessee Child Health Record form may be obatined from
your doctor or the Health Department. Sevier County Health Department
is located at 227 Cedar Street or telephone at (865)453-1032.
13. Be sure to obtain a written record of all immunizations, including
the dates the vaccine was administered, and signed by the health
provider. This record must be on the permanent Tennessee Child Health
Record form signed by the provider (physician or public health nurse).

14. REMEMBER - NO PUPIL WILL BE ADMITTED TO ANY
TENNESSEE SCHOOL UNTIL A CURRENT RECORD OF HEALTH EXAMINATION AND A
RECORD OF IMMUNIZATIONS HAS BEEN RECEIVED BY THE SCHOOL. SCHOOL HEALTH
RECORDS ARE SUBJECT TO ANNUAL AUDIT BY THE TENNESSEE DEPARTMENT OF
PUBLIC HEALTH. STUDENTS ENTERING KINDERGARTEN MUST HAVE A PHYSICIAL
AFTER JANUARY 1 OF THE YEAR THEY REGISTER.
